MCIL Long COVID Community Survey

This Long COVID survey is being conducted in partnership with the Metropolitan Center for Independent Living (MCIL), Access Press, and the Minnesota Department of Health (MDH). MCIL is a 501(c)3 statutory nonprofit with a mission to advance the independent living of people with disabilities by removing barriers and promoting choices. Fifty percent of the MCIL staff must be persons with disabilities. The funds for conducting this survey are a part of a statewide initiative to better understand the impact of Long COVID across diverse communities in Minnesota.

The goal of this survey is to learn more about awareness, beliefs, and experiences related to Long COVID within the disability community. All responses will remain confidential. Only MCIL and supporting evaluation staff will have access to your identifiable information. No identifying information will be shared in reports or publications.

Participation is voluntary and takes approximately 15 minutes to complete. You can answer any, all, some, or none of the questions with no effect on your relationships with MCIL, Access Press, and MDH. This information will guide MCIL to provide support to individuals and families living with Long COVID.

Eligibility and Background Information

Fatigue (persistent tiredness or exhaustion that doesn’t improve with rest)
Cough (Ongoing cough)
Shortness of breath (difficulty breathing or feeling breathless during minimal activity)
Brain fog (difficulty concentrating, remembering, or thinking clearly)
Muscle or joint pain (ongoing aches or stiffness)
Chest pain or tightness (discomfort or pain in the chest area)
Sleep disturbances (trouble falling asleep, staying asleep, or poor-quality sleep)
Loss or altered sense of smell or taste (difficulty smelling or tasting, or changes in perception)
Headaches (frequent or severe head pain)
Dizziness (feeling light-headed when you stand up or change positions)
Heart palpitations (feeling of rapid, irregular, or pounding heartbeats)
Depression or anxiety (strong feelings of sadness or worry)
Symptoms that get worse or reappear after mild to moderate physical or mental effort (post-exertional malaise)

Mild symptoms are those that were noticeable but did not cause much trouble. Severe symptoms are those that required you to seek medical assistance like going to urgent care, the emergency room, or being admitted to the hospital.

Long COVID refers to symptoms or health problems that occur after COVID-19 infection and are present for three months or longer. The symptoms may be new, persist from the initial infection, or may come and go. They can affect nearly any part of the body. Sometimes long-term symptoms are severe enough to interfere with daily life.
