Benefits Coaching Program Participant Satisfaction Survey

We invite you to complete a satisfaction survey on the recent work you did with MCIL staff. We may have helped you by exploring how your disability benefits could be affected by work, addressing your basic needs like housing or food, building skills for employment, or through other independent living services. This short survey should only take about two minutes of your time. Your answers will help us learn more about what we’re doing well and where we need to improve.
